Our program
was born out of a settlement in the Staley v. Kitzhaber case
and establishes a brokerage where state services are provided with
federal financial assistance through the Medicaid program. Under
this new program citizens with developmental disabilities waiting
for services will be assisted more quickly and be able to determine
what their most immediate needs are. The ISN sub-contracts with
the State of Oregon Department of Human Services in the implementation
of this program.
The ISN Support Services Brokerage is a leader in the transformation
of our health and human services and believes in the ability of
people to determine and direct their own lives. Through the ISN,
the citizen makes the determination of what kind of services they
need. Citizens are entitled to in-home or personal support services
such as personal care, job coaching or employment supports, in-home
staffing, respite care, community inclusion activities, minor environmental
adaptations, or other supports that assist the individual to live
and work in the community.
The Integrated Services Network is a community
organization formed by two not-for-profit organizations: Catholic
Community Services and Chehalem Youth and Family Services ISN members
have pooled resources into joint service contracts in order to improve
the quality and cost effectiveness of the service delivery.
Currently the ISN is operating in eight of Oregon's
counties; Marion, Benton, Clatsop, Columbia, Lincoln, Polk, Tillamook,
and Yamhill. Between January 1st, 2002 and June 30th, 2007 ISN will
serve 740 citizens with developmental disabilities.

You are at the Drivers Seat:
As a customer of the ISN, you have the responsibility
to ensure the quality and success of your self-directed plan. Your
Personal Agent can help you to understand and be successful with
your responsibilities. Because you are the boss, you have the responsibility
to:
Direct the creation of your plan. You
are the one who knows what you need and want.
Understand the limitations of your plan.
Make written changes to plan as needs
change and/or are met. Until you have made changes, you must follow
it as written.
Choose and supervise the people who work
for you.
Approve timesheets and invoices of the people who work for you.
Be aware of the funding in your plan and
make sure it is used properly.
Personal Agent Core Functions:
Assist individuals with DD to determine
their needs, plan Support Services in response to these needs,
and develop individualized budgets based on available resources
Assist individuals with DD to find and
arrange the resources necessary to implement planned Support Services.
Assist individuals with DD in assuring
the effective implementation of their plans over time, and help
make adjustments to the plan or plan goals as necessary
Provide information, education and technical
assistance for individuals with DD in order to help facilitate
effective plan implementation.
Act as a general fiscal intermediary in
the receipt and accounting of certain funds on behalf of an individual
in addition to making payment with the authorization of the individual,
and accounting for certain support plan costs.
Act as an employer agent in assisting
individuals with DD in fulfilling their roles and obligations
as employers of support staff when plan calls for such arrangements.
Facilitate development and expansion of
community resources.
Assure and assist individuals with DD
in monitoring of the quality of their support services.
As an ISN Customer, you have the right
to:
Not to be discriminated against. All people
eligible for services can use them as long as there is money and
room in the program regardless of age, race, skin color, sex,
religion, where you were born, how much money you make or how
long you have lived in Oregon.
A person centered assessment and plan. Your plan must state when
your support will start, how long it will last and cost. The plan
must state what other supports you are receiving or will receive,
a complete budget, and meeting dates to ensure that you are satisfied
with your plan.
A nursing care plan. If you need help from a nurse, a nurse will
complete information and instructions on the support you may require.
Review your plan. You have the right to review your plan with
your Personal Agent at least once per year to determine how well
it is working. Your plan can change if your needs change.
Change Personal Agents. You have the right
to request a different Personal Agent. We will do everything we
can to ensure that you have a Personal Agent that is a good fit
for you.
Complain or grieve decisions made by the
ISN. You can formally complain if you are unhappy with decisions
or how things are going
Be safe from abuse. ISN employees are
required to alert authorities if they observe signs of abuse.
Move to another county in Oregon without
losing your services.
Your Personal Agent will help you transition
your plan if you move you another county in Oregon. Your plan
will remain in effect until the date it was supposed to end.
